Excell Hospice works to promote improvement in a patient's quality of life, by ensuring comfort, pain and symptom management, as well as providing emotional and spiritual support to patients and their families through the end-of-life journey.
An individualized plan of care for each patient ensures dignity and respect through a holistic and comprehensive host of services, all coordinated with your existing physician and/or our medical team.
Hospice services can help patients with any life-limiting condition including cancer, heart disease, kidney disease, respiratory conditions, neurological disease and more. Excell is committed to partnering with each of our patients and their families, providing comprehensive support, education and skilled care.
Services can be provided in a number of settings – private residences, assisted living, skilled nursing facilities, nursing homes and independent living communities.
The Excell Hospice Team works directly with a patient's existing physician(s), and in conjunction with our medical directors to ensure the highest level of care. Once enrolled, Excell Hospice will provide you with experienced clinicians and resources to promote the highest quality of life surrounded by the people and the things that you love. Additionally, our on-call nurse is available 24 hours a day.
We recognize that medical care is not the only part of a hospice patient's needs. Our team also provides emotional and spiritual support to all who are part of this journey.
Excell's Hospice Programs and Services
Excell Hospice’s specialty services give you the best quality of life possible, when time is most precious. Our services range from specialized palliative care to simple companionship programs, all aimed to provide respect, dignity, comfort and support.
Palliative Care Program
Excell's Palliative Care Program is a collaboration of both Home Care & Hospice. The program's goals are to achieve patient comfort, enhance symptom management, improve quality of life, provide support for patients and their caregivers, and reduce the number of avoidable hospitalizations. Palliative Care is a medical specialty that focuses on the relief of pain and other symptoms to improve the quality of life for patients with a chronic, serious or life-limiting illness. The program introduces an interdisciplinary team of professionals that emphasize palliative services.
Excell Hospice's Q4 Program provides service when it's needed most. When a patient becomes imminent or is experiencing a symptom crisis, the Excell Hospice Team will provide a point of contact every 4 hours to monitor the patient's changing status. The team will notify the patient's attending physician of their condition, order medication and put into place the appropriate comfort measures.
Excell's Hospice serves not only our patients, but their families and caregivers as well. Our caregiver support staff and / or a chaplain offer one-on-one sessions for those who might benefit from individual counseling and support. Excell Hospice can also assist with respite care, funeral arrangements and other end-of-life decisions.
Excellent Days Program
Our hospice patients enjoy celebrating occasions with their friends and families. Excellent Days, coordinated by the Excell Hospice team, is designed to enrich our patient's lives by helping their wishes come true. Excellent Days offer hope, love and strength to all who participate.
Annual Tree Lighting Ceremony
The holidays often bring on unexpected emotion and grief for those who have lost a loved one. Excell Hospice’s Annual Tree Lighting Ceremony aims to help keep the spirit of those who have passed part of the holiday season. Participants are invited to bring an ornament that represents their loved one and then share their story as we decorate the tree. The Annual Tree Lighting Ceremony has become a healing event for families and friends, offering a safe place to both grieve and celebrate their precious memories.
Annual Butterfly Release
Each spring, Excell Hospice invites families and friends to gather, share a meal and remember those who have passed. Participants are given a butterfly in memory of their loved one and through a meaningful ceremony, release their butterfly and celebrate the lives of those who have died.
Veteran Pinning Ceremony
Excell Hospice honors each of our veteran patients with a Veteran Pinning Ceremony, acknowledging their service and sacrifices. Veterans are encouraged to share their stories while the Excell team, community members and others celebrate their commitment and thank them for their service.
Excell Hospice offers a Pet Therapy program to our patients and their family members. Excell Hospice's certified, trained pet and handler visit for 20-30 minute sessions where patients can pet or play with the dog and chat with the trainer. Pet therapy has been proven to improve social and emotional well-being and it is one of our favorite hospice programs.
Massage therapy sessions for Excell Hospice patients and their families promote relaxation and improve their sense of well-being.
Volunteers serve a priceless role as part of Excell's hospice team. Our volunteers share their talents and interest with patients, providing comfort, support and joy for those we serve. Grief Support Group
Excell Hospice offers monthly support groups for individuals grieving the death of a loved one. Facilitated by grief support staff and / or a chaplain, these groups create a safe environment in which family members and friends can express their feelings of grief and mourning, and begin to cope with their loss.
Specialized Grief Programs for Children
The grief process for children is unique and they often need more help to cope and heal. Excell Hospice has designed a specialized grief program for children to help them understand their losses while providing a safe place for them to express their feelings and emotions.
